• If your insurance requires a prior authorization or a referral, please contact your primary care physician to obtain it and please bring it to your visit.
• If we send your lab work or pathology to an outside laboratory, then you will receive a separate bill from them.
• Please keep in mind that skin exams and full body checks are considered by your insurance as non-routine exams and not as preventative services.
• Any services that are not covered by your insurance, are considered your responsibility and will be charged to your Credit Card on file.
• We can make arrangements for a monthly payment plan but this must be done prior to the actual procedure.
