First Visit

Please assist us by providing the following information at the time of your consultation:

• Your referral form if provided by your referring doctor.
• A list of medications you are currently taking and their doses.
• If you have medical insurance, please bring the insurance card.
• A valid state ID or driver’s license.

IMPORTANT: A parent or guardian must accompany all patients under 18 years of age at the first visit.  For subsequent visits, we can provide a Minor Consent form that allows us to see the patient without an adult.
