Welcome to the INSTITUTE of Dermatology & Aesthetics! On the day of your appointment, please arrive 15 minutes prior to your scheduled appointment time and present the following information to our front desk staff:

• Valid State ID or a Driver’s License
• Current insurance card: Without proof of active insurance, full payment is expected at the time of service.
• Your preferred pharmacy and list of current medications including dosages.
